1) Your CA Drivers License or CA Identification Card (REQUIRED)
2) Any medical records you have along with any medications that you take or have taken in the past. Doctors at our medical clinic will still evaluate you. It is understandable that some patients have no medical history or simply cannot locate their medical records.

As long as you have a condition that qualifies, you are eligible for a medical recommendation under law, and our doctors will see to it that you are provided with the relief necessary for your medical condition. See Qualifying Conditions for more information. Note: this is only a partial list of qualifying conditions as the law states:

  • Any other chronic or persistent medical symptom that substantially limits the ability of the person to conduct one or more major life activities, as defined by the Americans with Disabilities Act of 1990; or, if not alleviated, may cause serious harm to the patient’s safety or physical or mental health.

Many common conditions such as insomnia, anxiety, and chronic pain will qualifying as well.

How can I get my physician to send me my records?

Contact your doctor or the hospital where you received treatment.

Let them that you will be requesting your medical records. They will then tell you what to do.

Generally, you will need to provide a written authorization to release your records. We will need to ask for your doctor’s fax number and then fax him or her a release of medical records form.

Please make the effort to bring in some kind of medical documentation- this can mean any prescriptions, letters from your physician, medications, or anything that shows that you have been to a physician and have been diagnosed with your condition.

Who does the medical evaluation?

All of our medical evaluations are conducted by a California-licensed physician with over 25 years of medical experience.

During the initial visit, our licensed physicians will spending time examining medical history, performing a physical exam and determining the best medication for his or her condition, with regard to alternative medicine.

Although you will be medically evaluated by a CA licensed physician, the employees of Open Care’s Orange County Medical Clinic, including physician’s assistants, nurses, and other staff are not physicians and do not practice medicine or dispense any medical advice. The staff here to support and help physicians administer a clinic.
